Yes, you should. Anywhere you worked (even if it was for only five minutes) needs to send you a wage confirmation statement for the year. This will be either a W-2 (for regular or temporary employees), or a 1099 (for contractors). If you hadn't received your W-2 by February 15th, you should have contacted them immediately.

If you receive $1 or more of income a W-2 is mandatory. Call them and ask what the problem is. That should have been mailed out by Feb 1st this year.
